Home Care Services

MultiCare Home Health provides in-home personal assistance at its best under the direction and supervision of Registered Nurses and Rehabilitation professionals. When a change in health creates challenges with managing personal care, grooming, dressing and other activities of daily living, Home Health Aides and Personal Care Assistants can help make these tasks less cumbersome. Whether a patient needs assistance on a daily basis or several times a week, our team of trained paraprofessionals will help ensure the patient’s personal care and safety requirements are met in the comfort of their own home.

Home Health Aide services are generally provided as an integral component of a comprehensive home health plan of care, ordered by a physician, and covered under a Medicare, Medicaid or private insurance plan, in accordance with eligibility and coverage criteria.

Long Term Living Program

MultiCare Home Health is pleased to be a qualified provider of Personal Care Assistance Services through a contract with the Pennsylvania Department of Human Services, Office of Long Term Living, which includes the following programs:

  • OBRA Waiver
  • Attendant Care/Act 150
  • Community HealthChoices Providers

Personal Care Assistance

These programs facilitate invaluable home and community-based services which support people with physical and developmental disabilities, and those who are clinically eligible for nursing facility care, allowing them to live in the community and remain as independent as possible. Not part of a traditional home health plan of care, Personal Care Assistance services may include:

  • Assistance with self-care such as grooming, bathing, dressing and toileting
  • Enabling safety at home by assisting with ambulation, transfers such as bed to wheelchair or wheelchair to toilet, and fall prevention
  • Assistance with meal planning and preparation, light housekeeping, laundry and medication reminders
  • Companionship and engaging with a patient in hobbies and activities
  • Supervision for a patient with dementia or Alzheimer’s Disease
